Your Right to a Good Faith Estimate

Knowing what care will cost is part of feeling ready to begin. Here is how estimates work at Appleman Nutrition.

YOUR RIGHT

What the law provides

Under the federal No Surprises Act, anyone who is uninsured, or who is not using insurance to pay for care, has the right to receive a Good Faith Estimate of what that care is expected to cost.

Appleman Nutrition is a private-pay practice. We do not bill insurance, so this right applies to every client we work with.

WHAT YOU RECEIVE

What your estimate includes

Before your first appointment, you will receive a written Good Faith Estimate that sets out:

  • The fee for each type of session
  • The services we expect to provide, based on what you have shared with us
  • How and when you are billed

Each appointment is billed on its own. We do not sell packages of sessions, and we do not ask anyone to commit to a minimum number of visits.

HOW LONG CARE LASTS

Why the estimate does not predict a number of sessions

Nutrition care, and eating disorder care in particular, is not a fixed course with a known end date. How long someone works with us depends on what we learn together, how the work unfolds, and what each client decides is right for them.

For that reason, we do not estimate a total number of sessions before we have met you. What we can tell you is exactly what each session costs, so you always know what you are agreeing to.

ASKING FOR AN ESTIMATE

How to request one

Your estimate arrives with your intake paperwork, before your first appointment. You can also ask for one at any time, before or during care, and we will provide it in writing.

If our fees or the services you receive change while you are in care, you will receive an updated estimate.

IF A BILL IS DIFFERENT

If a bill is higher than your estimate

If you are ever billed substantially more than your Good Faith Estimate, you may be able to dispute the bill. You can learn about your rights and the dispute process at cms.gov/nosurprises, or by calling the No Surprises Help Desk at 1-800-985-3059.

QUESTIONS ABOUT COST

Fees are always part of the conversation
